Please use the forms below to start new service, make adjustments to your account, and apply for permits. These forms can be delivered in-person at City Hall during operating hours or mailed to the following address: PO Box 219, Bethany OK 73008.  If you need to change or update your account information, please call Customer Service at 405-789-2146 for instructions. Account changes may require you to fill out a form and/or supply documentation. Customer Service is available to answer your questions Monday through Friday 8:00am to 5:00pm.

New Service Applications must be delivered in-person to Customer Service, Monday through Friday 8:00am to 4:15pm. Instructions for starting new service can be found here. Garage Sale permits must also be delivered in-person, with the $10 fee, during regular operating hours.
